Hours:  8 am – 4:30 pm, Monday through Friday; other times by appointment

Fee:  No out of pocket cost for health promotion services

Insurance:  Most insurances accepted

Eligibility Information:  Rice County resident; assessed need for services

Capacity to Accommodate Clients with Limited Financial Resources:  No one is denied services because of inability to pay; no fees for health promotion services.

Capacity to Accommodate Clients who Speak Languages other than English:  Spanish, Somali, Nuer, Russian, and Ukranian language interpretors available; language line available.  Written materials available in English, Spanish and Somali

Services Offered:  Education/Awareness;  Prevention;  Counseling – Visits by Public Heatlh Nurses and Parent Aides;  Social Work and Public Health Nurse Case Management
